Evan Michael Beach, 3 months, passed away Sunday afternoon, June 12, 2016 at the University of Michigan Hospital in Ann Arbor.

He was born on March 21, 2016 to Derek Beach and Amber Perkins.

Survivors include his parents; Derek & Amber of Sycamore and he was preceded in death by his twin sister, Hayzel Audrey Beach on March 26, 2016.

A private burial was held for family at St. Mary Catholic Cemetery.

Memorial contributions may be made to a charity of the donor's choice.

Hoffmann-Gottfried-Mack Funeral Home & Crematory is assisting the family with arrangements.
