Hayzel Audrey Beach, 5 days, passed away Saturday morning, March 26, 2016 at Toledo Children's Hospital.

She was born on March 21, 2016 to Derek Beach and Amber Perkins.

Survivors include her parents; Derek & Amber of Sycamore; and her brother, Evan Michael Beach of Sycamore.

A private burial was held for family at St. Mary Catholic Cemetery.

Memorial contributions may be made to a charity of the donor's choice.

Hoffmann-Gottfried-Mack Funeral Home & Crematory is assisting the family with arrangements.
