Melvin P. Hook, Jr., 51, of Tiffin, passed away Tuesday morning, February 15, 2011 at St. Catherine's Care Center in Fostoria.

He was born on May 20, 1959 to Melvin P. and Betty J. (Annon) Hook, Sr. He married Donna J. McGee on June 12, 1982.

Survivors include his wife, Donna; 3 sons, Chad Everett Hook, Melvin P. Hook III, Preston Lee (Chelsey) Hook all of Tiffin; brothers, Howard of Dover, OH; Melford (Sam) Hook of Fort Seneca; Robert (Starla) Hook of Tiffin; David Hook of Fla.; sister, Cindy (Skip) Fahle of Fostoria; and10 grandchildren.

Melvin was a truck driver his entire life and also drove Taxi cab.

There will be no visitation or services at this time.

Memorial contributions may be made to the Mercy Cancer Center or to Bridge Home Health and Hospice.
